To avoid weeks of delays in getting access to medical cannabis at the Utah dispensaries please completed the following before your appointment.

Deliver your qualifying medical notes to us:
Bring documentation of your qualifying condition to the appointment
Fax it to 860-413-0862.
Log in to your patient portal and download the documents. If you are comfortable emailing your records you can email them to wbeesley@restorativehealthprimarycare.com
Take a picture of your Utah ID and text us at 385-449-0565 or have your ID with you at the time of the appointment. If you don't have your ID you'll need to show proof of residency. Utah Residency Verification - Current mortgage or rental contract; Major credit card bill (dated within 60 days) Property tax notice (statement or receipt dated within one year); School transcript (dated within 90 days)
Complete the steps on the State Web Site to have your portion for the medical cannabis card done prior to the appointment
Pricing
$130 - Cost of Initial medical evaluation
$95 - Each follow-up evaluation
$45 cancellation fee if cancellation occurs within 24 hours of appointment
