You must be fasting for at least 4 hours for the most accurate results

You must print out the requisition form that we will email to you. Some labs may allow you to show them a digital copy. If your driver's license doesn't match up with the requisition form please contact us because it will need to match otherwise they will refuse to draw your blood.

No additional payment will be required at the time of the blood draw.

Optional $50 fee - Contact our case management team for help finding lab draw locations covered by your insurance. Best for patients unsure how to get their labs drawn and which labs are covered by their insurance.
